·          Facet Joint Syndrome

But ultimately, it is likely to be Muscle Imbalance which has led to that diagnosis. This is because tight and weak muscles increase inappropriate stresses across the structures of the lower back and as a result of this, the facet joints may become inflamed, a disc may begin to bulge or you may just speed up the wear & tear process. Whichever it is, it is likely that the underlying problem is muscle imbalance. 

  

Surgery is NOT needed 

 

Nearly all of the Low Back Pain suffered by the general population DOES NOT NEED SURGERY as long as it is treated correctly. So what is Muscle Imbalance? 

  

Well, in an ideal world, all of the muscles in our body would work in complete harmony together, with some muscles playing the role of mobiliser (where they allow day to day movements to take place) and others taking the role of stabiliser (where they provide support and stability). 

  

Unfortunately this is very rarely the case. Due to our own specific make up or habits we have adopted over the years, some of these muscles tend to become weak, while others become too tight. It is this weakness and tightness which is being referred to when we talk about Muscle Imbalance.

  

  

Weakness: 

If there are muscle present which are weak, they will not be able to provide the lower back with the support it needs. Consequently, even day to day activities will be placing increased stress across the joints and soft tissues of the lower back, potentially resulting in pain. 

  

  

Tightness: 

Tight muscles on the other hand will be 'pulling' or 'tugging' too much on the structures they are attached to. Once again, this increased stress being placed across these structures, for our reference the low back, will lead to pain. 

  

Weakness & Tightness: 

This is just what we do not want. If you have weak muscles present which are not able to provide stability to the structures of the lower back, yet you also have tight muscles that are pulling to hard on those same structures, Low Back Pain is almost inevitable! 

  

Therefore, in order to address these muscle imbalances which are causing you Low Back Pain, an appropriate exercise programme is needed.  

  

  

Is an exercise programme enough?  

 

My first response to that question would be that an appropriate exercise programme is nearly always needed. This is to achieve the goal of resolving any muscle imbalance which is likely to be causing you your Low Back Pain. However, to answer whether it is enough, I would say probably not. 

  

My reasons for saying this, is because the human body has an innate desire to heal itself of any injury it has suffered 24 hours per day - 7 days per week. Therefore it is VITAL we provide it with the correct conditions to do this 24/7.

  

Suffering with Low Back Pain is no different in principle (although probably a lot more painful) than any other injury we suffer. Let's take a torn muscle for example... 

  

If you had torn your hamstring muscle, been to see a Physiotherapist who treated you and provided you with an appropriate exercise programme, there should be no problems with the muscle healing itself 100%. Why? Because that is what the body is fantastic at. 

  

However, if you received the same treatment and performed the same exercises BUT you continued to play sport / run around, I am sure you would not be too surprised if you re-injured your muscle or it at least took a lot longer to heal. This is simply because you would not have been giving it the ideal conditions to heal itself 24 hours a day. 

  

This is exactly what happens with Low Back Pain. Even if we are receiving treatment for it, if we do not provide the body with the correct conditions to heal itself all day, every day, at best it will take a lot longer to heal, at worst it will not heal and your pain levels may even increase. 

  

With regards to your Low Back Pain, this may take the form of being too active and trying to do too much too soon, or the opposite where we are frightened to do anything and therefore very little activity is performed. In the latter example, your low back would gain no strength as it healed, a perfect recipe for re-injury! 

     

Therefore, can you see how, even if you are performing an appropriate exercise programme, your low back pain may take much longer to heal if you are not providing it with the optimum conditions to heal itself all day, every day. 

  

All day includes sleeping as well. On average, we probably spend a little less than 1/3 of our time sleeping. If you are sleeping in a position which is not promoting optimal healing, or if you are carrying out activities before going to bed which are aggravating your pain, this time spent in bed is wasted healing time. The most disappointing thing about this is that...
